Comprehensive Breast Explantation is a procedure in which Dr. Mirmanesh removes the breast implant and capsule followed by a breast lift and fat transfer to improve the size and shape of the breast, when compared to just removing the implants and capsule.
In removing the breast implants and capsule, Dr. Mirmanesh makes every attempt to perform a total capsulectomy in which complete removal of all of the capsule and implant with the capsule intact is performed. The goal is to not open the capsule during the surgery. Dr. Mirmanesh makes every attempt to perform this while using the smallest incision possible.
After Dr. Mirmanesh removes the implant and capsule, remaining skin and fat will be reshaped to create an improved breast contour. After proper and thorough evaluation, Dr. Mirmanesh will make choose an incision pattern that is right for you.
There are two basic types of incision patterns for the breast reshaping.
- The Vertical (Lollipop) incision is created around the areola and vertically down to the breast crease, and is ideal for those with a mild to moderate amount of breast skin excess.
- The Inverted-T (Anchor) Incision is created around the areola, vertically down to the breast crease, and then horizontally along the breast crease. This incision is best for those with a significant amount of breast skin excess.
After the breast is successfully reshaped, fat transfer in which fat is extracted from a site on the patient’s body and is then reinserted (transplanted via injection) into the breast. Fat transfer cannot be rejected or cause an allergic reaction because the fat is not a foreign substance and completely their own. The liposuction portion of the fat transfer process will be performed with a form of Tumescent Liposuction. Other techniques like laser liposuction or ultrasonic liposuction kill the fat cells to facilitate their removal and although this is okay for achieving body sculpting alone, fat transfer depends on the successful removal of living fat cells. The liposuction portion can enhance one’s body shape as well. The liposuction will leave behind very small scars a few millimeters in length that should heal to be nearly invisible.
